So actually, what happened is that the first person clearly ordered water, also, the second person wanted to order water too but he used this phrase “I will have H20 too”, the waiter mistook the “too” to “2” which can now be deciphered as “H202” and H202 is Hydrogen peroxide which is the active ingredient for making soap, bleaching agent and even first aid antiseptic.

This H202 is not edible and can take a life.

Technically, the waiter misheard the “h20 too” to “h202” and subsequently gave him h202 and he consumed it and died.
